Requirements for leaving Japan

Below are the requirements for leaving Japan with your pet. This will vary a bit depending on where you are currently located. You should also check with the country you are entering to view their requirements. The below requirements are for entering the continental United States from Japan.

  • Current rabies certificate 
  • Health certificate dated within 10 days of your flight
  • Green tag from quarantine 
    • For pets in Okinawa you will go to Naha 1-3 days before your flight with your pet, health certificate, rabies certificate, flight numbers, orders (if you have them) and ID. They will stamp all of your paperwork and give you the green tag
    • For pets on mainland Japan (Iwakuni, Misawa, etc) quarantie check out can be done at your nearest quarantine facility or in Narita/Haneda Airport

USA or Japan to Hawaii

Below are the requirements for leaving the USA and entering Hawaii with your pet​.

Below are the requirements for leaving the USA and entering Hawaii with your pet​. 

  • 2 seperate rabies shots administered at least 30 days apart
  • ​The most recent rabies must not be expired upon entry and must have been administed at least 90 days before entry
  • Microchip implantation must be done before the FAVN blood draw ​
  • ​FAVN blood draw (results will be mailed directly to Hawaii from the lab)
  • 120 days must have passed after the lab received your pets blood sample
  • ​Original ink signature or carbon copy rabies vaccination certificates for the two most ​recent rabies vaccinations the pet received are required. (Photocopies and computer generated certificates are not accepted.) ​
  • ​The vaccination certificates must have the vaccine name, lot or serial number, booster interval, vaccination date and vaccine lot expiration date listed. 
  • ​An original health certificate in English, done within 14 days of arrival in Hawaii including rabies ​vaccine name, lot or serial number, booster interval, vaccination date and expiration date is required. 
  • ​A veterinarian must treat the pet for ticks with a product containing Fipronil or an equivalent long-acting product labeled to kill ticks (Revolution® is not acceptable) within 14 days of arrival and the product name and date of treatment must be recorded on the pet’s health certificate. ​
  • ​Dog & Cat Import Form AQS-278 (click here for this form)
  • ​Payment of fees ($165 per pet in advance for direct airport release or $224 for 5-day-orless program) by cashier’s check or money order (no personal checks are accepted). Included your pet’s microchip number with the payment to ensure proper credit. Payable to: Department of Agriculture
  • ​You must arrange for your pet to arrive at the Airport Animal Quarantine Holding Facility at the Honolulu International Airport during normal inspection hours between 8:00 AM to 5:00 PM. It may take up to one hour for the airlines to transport a pet to the Airport Animal Quarantine Holding Facility [Phone: (808) 837-8092] and animals not arriving at the facility during normal inspection hours will not be released at the airport.

Your pet must complete the 120-day waiting period before arriving in Hawaii or your pet will not qualify for either direct airport release or the 5-day-or-less quarantine program
